Your opinion is very valuable to us at PS 110 and District 24. We would like to learn more about the needs of our school community, so that we can better serve our students and their families. To help gather this information, I am asking that you take a moment and complete the Parent Centered Survey of only 13 questions. Your help with this survey is greatly appreciated!

Forms

Emergency Contact Information (Blue Cards)

It is extremely important that P.S.110Q has the current, correct information for contacting parents, guardians or persons in parental relations, at all times – especially during the school day. It is the responsibility of Parent/Guardian to see that the Blue Emergency Contact Card, which is kept on file in the main office, is completely filled out and to date. Notify the school of any changes. Read more for complete emergency contact information.

Student COVID-19 Testing (Consent Form)

Monthly testing of randomly selected staff and students is a vital part of our efforts to prevent COVID-19 transmission in our buildings, because it helps identify positive COVID-19 cases when symptoms are not present. Read more for updates and consent form.

Health Screening Questionnaire

To enter our building you must fill out the Health Screening Questionnaire by the Department of Education. All staff, students and guests must complete their individual screening using the web tool or submitting a printed copy. Questionnaires are available in different languages. Read more for all questionnaires.
